Information you voluntarily provide to us:  We have a duty to protect health information about you.  The patient intake form is HIPPA protected and secured through a third party site.  By completing the intake form or the contact us form you are giving us permission to contact you via email, regular mail, or telephone.  We do not share or sell your personal information.

Information collected passively:  Google Analytics is used to analyze our website and visitor behavior.  The information may include IP addresses and page tracking.  We may collect statistics about the behavior of visitors to the website.  We do not disclose personally identifying information.
